In-Network Contracts

Although Sema4 is in-network with a majority of regional and national commercial health insurance plans, Sema4 may not be in-network for all health plan types, in all geographies, or for all health plan network products. Ordering providers and patients are encouraged to check the patient’s health plan directory or inquire with their health plan for network status of any provider for which they are ordering services, ahead of those services being rendered.

Network status is not a guarantee of coverage. Health plan requirements must be met for coverage to apply and for patients to access services at their in-network benefit levels. Obtaining prior authorization and meeting health plan medical policy medical necessity are examples of requirements that may need to be met.

Network status with a commercial health insurance plan does not mean that patients will have no out-of-pocket costs for testing. Patient coverage for services and out-of-pockets costs are subject to health plan benefits, medical policy, and any exclusions that may apply. Patients may be responsible for co-insurance or unmet deductibles and non-covered amounts.
